Hello. In the case of maternity allowance (and other rights you get when you have a baby), CSDs have a practice of labelling offices alphabetically, and this is how applicants are called. It doesn't matter if the office next door is empty, even for hours, and the employee has time to walk down the corridor and have a coffee while I wait for more than an hour for my turn in the office next door. You have to wait outside the office that bears the first letter of your surname. Personally, I also had a form with me that I printed out from the CSD website, but the clerk refused to accept it because it was wrong. So there is a form on the CSD website for claiming maternity allowance which is not correct. Please inform the relevant ministry, which should take action and check the implementation of the work in all CSDs.
